Start practicing! The muscles you've got to use to play (forming an embouchure, proper breathing, etc.) are probably out of shape, and the only way to get back into the swing of things is to practice. If you don't want your playing to break down too much, try not to go without playing (at least some scales or a piece or two) for more than 2 or 3 days. The results won't be as good as if you practiced every day, but at least you won't have to rebuild your playing completely.
