You should be able to find it in a "fakebook" (along with dozens of other tunes) quite cheaply. The easiest way might be just to transcribe it from a YouTube clip.
It's a 32-bar standard, AABA form, often played in the key of F. The "A" section is straight-ahead; the "B" section (or "bridge") is a bit trickier. Let's know how you go.
(i) For colour, you might prefer Am7(b5) (a.k.a. Ař ... "A half-diminished") instead of Am7. Trad/swing people will sometimes refer to this chord as Cm6.
(ii) You could use a D7(b9) if you didn't mind the momentary b9 (Eb)'s clash with the melody's D. A question of style/era/personal taste.
